Uninsured or
Insured but without drug coverage or
Insured but in need of off-formulary medications
Low income (< 250% Federal Poverty Level)

Must Provide
Original prescription (signed by physician) for each requested medication

Proof of income (Income Tax Statement, W2, Pay Stub, Bank Statement, etc.)
